These are our standard self-pay rates. Because some visits vary by complexity, your exact price is confirmed before your visit. Labs, imaging, and medications (if needed) are billed transparently and separately. Concierge membership is a direct-care arrangement and is not health insurance.

In Ohio? Use your insurance.

For Ohio patients, we'll bill insurance directly. We're in-network with Medicare, Medicaid, and most major plans including Aetna, Anthem (BC/BS), UnitedHealthcare, Humana, Cigna, Medical Mutual, Caresource, and Buckeye. For covered visits, you pay only your normal copay or deductible — not the self-pay price.

In Illinois, or prefer to self-pay?

All Illinois visits are simple self-pay — we don't bill insurance in Illinois. The same applies anywhere if you're uninsured, out-of-network, or just prefer to pay privately: our flat self-pay rates above are exactly what you'll pay. No claims, no surprise bills. We can also provide an itemized superbill you may submit to your own insurer for possible out-of-network reimbursement.

A note for Ohio Medicare & Medicaid patients: We accept Medicare and Medicaid in Ohio. Visits covered by your plan are billed to your plan — we won't charge you a self-pay rate for a covered service. Self-pay pricing applies to services your plan doesn't cover, or where paying privately is permitted. (In Illinois, all visits are self-pay.) When you reach out, we'll tell you exactly how your specific visit will be billed before you commit to anything.

In Ohio, likely yes — we're in-network with Medicare, Medicaid, and most major plans (Aetna, Anthem BC/BS, UnitedHealthcare, Humana, Cigna, Medical Mutual, Caresource, Buckeye, with Molina coming soon), and you'd pay your normal copay. In Illinois, we don't bill insurance — all Illinois visits are simple self-pay at the flat rates listed. Either way, we'll confirm before your visit.

If you're in Ohio, where we accept Medicare, we bill Medicare for covered visits rather than charging you a self-pay rate — that's the rule, and we follow it. We provide video visits to residents physically located in Ohio and Illinois, where the physician is licensed.

Yes, when it's clinically appropriate. After evaluating you, the physician can send prescriptions to your pharmacy. Certain medications have additional rules and may require specific steps or an in-person evaluation.

Often the same or next day. Evening and weekend appointments are available, and if our listed times don't work, reach out — we can frequently arrange something else.

Pay-per-visit means you simply pay for each visit you book — no commitment. Concierge is an optional monthly membership for ongoing patients who want direct access to the physician, priority scheduling, and proactive year-round care.
